Leaving Care and Pathway Plan Policy
This Leaving Care and Pathway Plan Policy has been designed to support full compliance with the Children’s Homes (England) Regulations 2015 and the associated Children’s Homes Quality Standards.
The policy provides clear, practical guidance for staff on supporting young people as they prepare to leave care and transition to independent living. It outlines the process for developing, reviewing and implementing pathway plans in partnership with the young person, local authority and relevant professionals. The policy emphasises promoting independence, emotional wellbeing, education, training and employment, housing, financial management and life skills, while ensuring safeguarding and ongoing support during and after the transition. It also outlines responsibilities for maintaining clear records, monitoring progress, and ensuring the young person’s voice is central to decision-making.
